Enrollment in Optum Pay is secure, simple and smart

If you’re a healthcare organization, you will need to have the following information to complete your enrollment online.

Provider information

To start, we'll need your organization name, mailing information and tax identification number (TIN).

New user information

To assign multiple users to your Optum Pay account, provide the contact information for each designated Optum Pay admin.

Bank information

To get funds into your preferred account, please include your routing and bank account information.

Financial information

Have a signed, dated copy of your organization’s federal W-9, and a voided check or bank letter for each account where payments will be deposited.
