Optum Rx is reinventing the PBM model

Fixed pricing, predictability and real-time visibility into medication costs are the core of the new Optum Rx pharmacy benefit management (PBM) model.

Optum Rx recently unveiled a new transparent pharmacy care model that fundamentally changes how pharmacy benefits are priced and delivered. This model represents a revolution not just for Optum Rx, but for the broader PBM industry.  

This announcement builds on our recent actions to modernize pharmacy care and improve affordability and access across the system. 

How transparent PBM models differ from traditional approaches

Optum Rx has taken an industry-leading stance to demonstrate value and deliver clearer, more predictable pharmacy experiences. While others in the industry have introduced elements of transparency, our model stands apart in several key ways. The difference is most clearly illustrated in how the new transparent model compares to industry norms:

This is not a refinement of the PBM model — it’s a reinvention.

New demands on PBMs

PBMs have long operated successfully within a complex ecosystem involving manufacturers, pharmacies, employers and patients. Traditional models that tied rebates, revenue and prescription volume to drug prices created a perceived conflict: Higher system costs could translate into higher PBM revenue.

But employers increasingly want greater visibility and more clarity. And Optum Rx has been listening. 

We have been expanding transparency for years by making financial flows independently auditable and providing greater visibility into rebate economics and formulary decisions through our Pharmacy & Therapeutics Committee. 

We have also: 

  • Committed to passing through 100% of manufacturer rebates by January 1, 2028
  • Transitioned all community and independent pharmacies to cost-based reimbursement models
  • Increased reimbursement rates for more than 2,300 unaffiliated pharmacies 

In parallel, we’ve reduced administrative burden by eliminating 33% of reauthorizations since 2025. 

What is a transparent pharmacy benefit model?

The newly introduced Optum Rx pharmacy care model represents a structural shift, not an incremental improvement. This model helps plan sponsors and employers improve predictability through fixed pricing and gives patients real-time visibility into medication costs — addressing some of the most persistent challenges in pharmacy care today.

At its core are 3 defining changes:

1. We are introducing a transparent, simpler pharmacy care model.

This market-leading approach builds on the Optum Rx multi-year commitment to transparency by fundamentally changing how pharmacy benefits are priced and delivered with end-to-end visibility into fees, including the group purchasing organization (GPO) with clear disclosure of payments received from pharmaceutical manufacturers. 

 

  • Clients receive full disclosure of manufacturer payments.
  • Fees are simple, stable and predictable. 

2. We are shifting toward a simpler, predictable pricing model.

The new monthly fee-based structure is independent of manufacturers’ list prices or prescription volume, eliminating spread pricing and similar practices to deliver greater clarity, predictability and affordability to pharmacy care. 

3. We are empowering patients with digital affordability tools. 

New digital tools like Shop MyScript give consumers upfront visibility into pricing and options before filling a prescription. This enables more informed decisions, reduces surprise costs and improves adherence at the point of care.

  • Clearer choices at the point of care
